Every first-time patient is required to fill out a set of new patient forms. You can access these forms by mail, online through our patient portal, or in person by arriving at your appointment about 15 minutes early.

For our patients’ convenience, we currently accept most major insurance plans (including Medicare) and cash payments for those without insurance.

You can schedule your virtual appointment through our convenient mobile app, Spruce. If you need to cancel an existing appointment, please contact us within 24 hours of your appointment time.

We care about the safety of you and your loved ones. Our staff follow all CDC-recommended guidelines for PPE use. We also provide an outdoor waiting area, as well as the ability to wait in your car and be called in when ready.

We do not currently offer COVID-19 vaccinations, but hope to be able to by the end of 2021.
